EXPENSE REFUND AGREEMENT

I acknowledge that my student organization will be responsible for providing an expense refund payment (check or money order) in the amount of $150.00 made payable to "University of South Florida".  Payment of the expense refund is to be submitted to the Office of the Registrar to account for all costs pertaining to the development and fulfillment of this data request. Payment must be received before the Registrar’s Office will begin development work on the request as well as release of the data.
